Thank you for your interest in requesting a quotation for group health coverage with NetCare. We currently do not offer individual health coverage.

Listed below are some important underwriting requirements pertaining to our group health coverage:

• All firms must be legitimate businesses on Guam , CNMI or Palau with a valid business license and compulsory Worker's Compensation Coverage.
• NetCare's group health products are available to firms that employ a minimum of two (2) or more full-time employees (working at least 20-30 hours per week) and can demonstrate that there is a clear employee/employer relationship.
• For groups employing less than ten (10) employees, a Wage and Hourly Report for the two preceeding quarters filed with the Department of Labor must be submitted to NetCare as proof of employment for each individual at the time of enrollment.
• Firms must have been in business for a minimum of two (2) years prior to enrollment with NetCare.
• Firms must also have prior group health or individual health coverage with another carrier in the past two (2) years prior to enrollment with NetCare.

Once you have fully completed the information requested, you can then click the SUBMIT button directly to NetCare's Marketing Department. A marketing representative will be in contact with you within 24 hours of receipt of your request.
